Application for a police clearance certificate

申請できるかた（Eligibility）

岩手県警察本部で申請ができるのは、次の方です。
Police clearance certificates by the Iwate Prefectural Police Headquarters are issued only to:

1. 岩手県内に住民登録している方。
   Residents of Iwate.(if you are a non-Japanese, who is now registered as a resident of Iwate.)
2. 既に海外へ渡航されている方で、住民票が除票になっており、日本における最終の住民登録が岩手県内にあった方。
   Ex-residents with Iwate as the very last registered place.

ご注意(NOTES)

• 岩手県以外に住民登録している方は、登録している都道府県の警察本部での申請となります。
  If you are not a Iwate-registered regident- You should apply to the Police Headquars of the prefecture where you live.
• 提出先国・発給事由によっては直ちに申請を受理できない場合がありますので、必ず事前に電話でお問い合わせください。
  Depending on what countries the police clearance certificate to and the reason why the certificate is required. There are some cases that the Police Headquarters will withhold the acceptance. In order to avoid such a time loss, please check with us in advance.

• 注：予約は必要ありません。
• 注：申請手続きに約30分程度かかります。

Closed on Saturday, Sunday, National holidays and Holidays at the end and beginning of the year.

• No appointment necessary.
• It may take about 30 minutes to fill out the application.

申請に必要な書類(Documents to be submitted)

岩手県内に住民登録をしている方（Applicants who have registered their residency in Iwate Prefecture）

1. パスポート（有効期限内のもの）
   Your valid passport (must be original, no photocopy)
2. 次のいずれか、
   住民票、住民票記載事項証明書（発行から6カ月以内のもの）、在留カード、特別永住者証明書、住民基本台帳カード、運転免許証（住民登録された住所の記載があるもの）
   One of documents such as: an official copy of your Resident's Card, a certificate of items listed in Resident Register (issued within the last 6mouths), Residence Card, Special Permanent Resident certificate, Basic Resident's Registration Card, driver's license.
3. 証明書の発給を必要としている事実が確認できる書類（要求機関発行の発給依頼書、記載済みのビザ申請書、入学許可書、出張命令書、雇用契約書、採用通知書など）
   Any necessary supporting documents for this certificate. (a written reqest from the institution which requires the certificate, an application from of visa which is filled in the necessary items, a document of admission to a school, an order of business trip, an employment contract, a notification of employment, for instance.)

国外に居住していて岩手県内に最終住居登録があった方（Overseas applicants who have registered their residency in Iwate Prefecture）

1. パスポート（有効期限内のもの）
   Your valid passport (must be original, no photocopy)
2. 現住所を確認できる書類（外国政府発行の運転免許証、郵便物など）
   Any document (ex. your local driver's license, postal matter) that verifies your present address.
3. 次のいずれか、
   住民票、住民票記載事項証明書、戸籍の附票（発行から6カ月以内のもの）、在留カード、特別永住者証明書
   One of documents such as: an official copy of your Resident's Card, a certificate of items listed in Resident Register, an official copy of Past Residential Record in Family Registry (issued within the last 6mouths), Residence Card, Special Permanent Resident certificate.
4. 証明書の発給を必要としている事実が確認できる書類（要求機関発行の発給依頼書、記載済みのビザ申請書、入学許可書、出張命令書、雇用契約書、採用通知書など）
   Any necessary supporting documents for this certificate. 証明書のお渡し方法(Receipt of the certificate)

1. 直接ご本人にお渡しします。郵送は出来ません。（代理受理希望の方は、申請時にお申し出ください。）
   A police clearance certificate will be issued directly to its applicant.
   If you wish someone else to pick up your certificate, please let us know when you apply.
2. 申請から発給までには1週間程度かかります。
   注：年末年始、ゴールデンウィークの期間は、通常より発給が遅くなります。
   Your police clearance certificate will be made available in about 1 week.
   However, the processing time may be delayed with the national holidays. For example: at the end and beginning of the year; during the series of national holidays in May.

申請にあたっての留意点(Notes)

1. 申請の際は指紋を採取しますので、必ずご本人がいらしてください。
   Applicants must apply in person as the procedure requires fingerprinting.
2. 現在、申請者が日本国外に居住している場合は、居住している国の日本大使館・領事館でも申請ができます。
   詳しくは、現地の日本大使館・領事館にお問い合わせください。
   If applicants live abroad, they can obtain the certificates from their local Japanese embassy or consulate of their countries (or regions)
   Please contact the Embassy (Cosulate) near you for further information.
3. 手数料は無料です。
   Issuing of certificate is free of charge.
